United Hospice has an incredible new opportunity for a Social Worker (LMSW or LCSW). We are seeking an experienced highly skilled, compassionate Hospice Social Worker to join our talented team to work in our Community Hospice. In this critical role as an integral member of our clinical team, you will visit patients and families in their homes throughout Rockland County. You are kind and mission driven. Working with a diverse patient population, you must be able to communicate effectively, while sustaining a caring, kind and professional disposition. The successful candidate will have strong clinical assessment skills and excellent written and verbal communication. You must possess the ability to accurately assess the psychosocial needs of patients admitted to our program. You will be at ease relating to a culturally diverse population while being adept at building a rapport with the terminally ill patient and their family/loved ones. You have experience documenting and using EMRs. You are adept at collaborating with a team approach and presenting at Inter-Disciplinary Team meetings.

The qualified candidate must have at least three years supervised direct practice experience in a health care setting or post-acute hospital services. Specific Hospice, or end-of-life care experience preferred. All Clinical staff participate in a shared weekend coverage schedule when each member of the larger team works one weekend day on a rotating basis. Bi-Lingual (Spanish-English) a plus, with additional salary enhancement at hire.
Daily access to an automobile is required. A valid NY or NJ State Driver’s License, ability to drive and driving record acceptable for agency insurance coverage are required. Because you will be working in our community-based program, you will be driving from patient residence to patient residence with mileage reimbursement for travel incurred.
