Performance Improvement Manager

United Hospice has an exciting new opportunity for a full time Performance Improvement Manager to support our growing organization. We have just begun our search for a highly skilled, experienced individual to join our team.  The Manager of Performance Improvement is responsible for the overall development and oversite of the quality improvement programs for the organization. The position provides expertise on hospice and palliative care market matters regionally and nationally. Candidate must demonstrate knowledge of clinical information and understanding of end of disease processes, assessment, and symptom management.

In this critical role you will manage compliance with Hospice Item Set (HIS), Joint Commission, Centers for Medicare and Medicaid (CMS) and NYS Department of Health regulations. As an established and highly regarded non-profit hospice in Rockland County, we are seeking someone with a passion for end of life care. 

As Performance Improvement Manager you will be an essential member of both the Administrative and Clinical teams, serving as gatekeeper of our annual Performance Improvement Plan (PI). You will manage compliance with Clinical Record reviews, Emergency Preparedness Plan and Quality Assurance-Risk Management.  

A self-learner capable of completing ongoing self-development. Responsible for assuring patient and staff safety.  Responsible for implementing strategic clinical goals and objectives as specified by annual plan and or new regulations. Responsible for managing aggregate and trend data collected to correct identified opportunities of improvements.

Must be skilled in the use of Excel, databases, EMR and manipulating data to generate management reports. Must monitor electronic medical record meticulously by daily audits of all internal and external service providers for effective interchange and coordination of service in providing care. Must demonstrate strong analytical and organizational skills necessary to accurately identify and analyze trends from audit results and communicate corrections. The manager keeps a daily log containing audit details and builds analysis to facilitate follow up activities. 

The position is responsible to conduct an ongoing, comprehensive, integrated agency assessment of the quality and appropriateness of the care provided, including services provided under contracts. Assists in the preparation for accreditation and/or certification surveys; ensure all assigned locations adhere to accrediting body guidelines. Responsible for maintaining services, systems and procedures to assure compliance with licensure regulations and standards. Must be proficient in developing, documenting, revising and evaluating programs and creating communication documents such as policies and procedures to achieve end goal of quality hospice service delivery. Prepare annual quality improvement report as part of annual agency evaluation. Ensure complete compliance to agency compliance standards and plan.

One of your essential job responsibilities will be to chair our Professional Advisory Board (PAC), facilitating quarterly meetings to review incidents, policies and internal protocol. You must be well versed with Joint Commission accreditation and Hospice assessment of Health Providers and Services (CAHPS). You are responsible for OSHA reporting and Corporate Compliance. Experience with submission of HIS data is strongly preferred. Hospice and PI experience is essential. Registered Nurse (RN) with an active NYS License and BSN required, Master’s in Nursing or Public Health preferred.  You are a skilled meeting chair and minute producer.

Your schedule is flexible with occasional weekend and evening hours as needed as you will participate in Administrative On-Call (AOC) responsibilities with clinical team rotation.
