United Hospice has new and exciting opportunities for compassionate and experienced Hospice Social Workers to join our talented team. As we prepare to expand our services to Orange County, we are seeking bilingual Social Workers to work full-time to meet the needs of both our Rockland and Orange County community based patients.
It is essential our employees have an understanding and commitment to hospice philosophy of care. Working with a diverse patient population, you must be able to communicate effectively, while sustaining a caring, kind and professional disposition. The successful candidate will have strong clinical assessment skills and excellent written and verbal communication.
You must possess the ability to accurately assess the psychosocial needs of patients admitted to our program. You will be at ease relating to a culturally diverse population while being adept at building a rapport with the terminally ill patient and their family/loved ones.
The qualified candidate must have at least three years supervised direct practice experience in a health care setting or three years prior experience with post-acute hospital services. Hospice, end-of-life care experience preferred. Weekend hours required. Must be bilingual (English-Spanish), with additional salary enhancement at hire.
Daily access to an automobile is required. A valid NY or NJ State Driver’s License, ability to drive and driving record acceptable for agency insurance coverage are required. If you are working in our community-based program, you will be driving from patient residence to patient residence with mileage reimbursement for travel incurred.
