Continuous
Care Nurse
Qualifications:
Physical Requirements:
-
Physical
agility with repetitive occurrence including but not
limited to: walking, running, bending, stooping,
climbing stairs, kneeling, crouching, reaching, sitting
and twisting

Lifting
greater than 50lbs repeatedly throughout the work day
-
Ability
to write, perform data entry and look at a computer up
to two hours daily

Vision
acuity correctable to normal with normal color
perception

Hearing
acuity correctable to normal

Acute
sense of smell for normal perception

to verbalize so that the average person can comprehend
Cognitive Requirements:

Responsibilities:

Give skilled nursing care for a minimum
of a four (4) hour shift to Hospice patients/families in
their homes, based upon the Hospice care plan

Teach, supervise and counsel the
patient/family regarding care related to the patient's
illness

Contribute as a team member to implement
that portion of the Hospice care plan relating to
continuous care in the coordination with other team
members

Document the care given in an accurate
and timely manner

Perform other related duties as required

patient and their families

Licensed
Practical Nurse
Qualifications:

Graduate of an accredited practical nursing
program

Possession of a valid and current NYS Licensed
Practical Nurse’s license

Minimum of 2 years experience in nursing

Complies with accepted professional standards
and practice

Commitment to the Hospice Philosophy of Care

Computer literate
-
Licensed driver with an insured automobile and
compliance with state standards
Physical
Requirements:

Prolonged or considerable walking or standing,
able to lift, position and/or transfer patients.

Able to lift supplies and equipment,
considerable reaching, stooping, bending,
kneeling and/or crouching.

Visual acuity and hearing to perform required

Provides direct patient care as defined in NYS

Qualifications:

Registered professional nurse
-
Licensed to practice nursing in
New York State
- One to two years
recent bedside nursing experience with good
clinical skills
- Some experience
caring for terminally ill patients and a basic
understanding and commitment to the hospice
concept

Experience in home care, hospice
and/or oncology preferred
- Ability to
assess the needs of terminally ill patients and
their families and to plan care to meet these
needs
- Basic knowledge
of medications, treatments and therapies for
pain and symptom control
-
Ability to communicate
effectively orally and in writing
-
Effective Oct.15,
2006, RNs (excluding per diems) will sit for
and pass the hospice and palliative care nursing
certification exam within three (3) years of
start of employment at UHR
-
Good listening skills
-
Ability to work within a team
concept

Must possess strong
clinical skills and a team orientation as well
as experience with pain/symptom management

Must possess car,

care needs and intervenes to ensure that needs
are met in an appropriate and timely manner:
provide symptom management and specialized
nursing skills related to end of life care.

Collaborates
with clinical and support staffs to effectively
coordinate the daily care of the patient:
informs physician and other personnel of changes
in patients’ needs and outcome of interventions.

Develops and
maintains effective relationship with patients
and families; educates patients and families
regarding disease process, self care techniques,
prevention strategies, end of life care process
and dealing with ethical issues.

Creates a
positive environment conducive to team building
and progressive nursing practice by

with professional, regulatory and agency
standards.
